PRIVACY REGULATION


    Praktijk CeeYu is responsible for the processing of personal data as shown in this privacy policy. Processing of personal data is done carefully in accordance with the requirements of the General Data Protection Regulation (GDPR). For proper treatment it is necessary that I, as your treating therapist/coach, create a file. This is also a legal obligation imposed by the Medical Treatment Contracts Act (WGBO/Wkkgz). Your file contains notes about your state of health and information about the coaching and treatments carried out. The file also contains information that is necessary for your treatment and that I have requested, after your explicit permission, from another healthcare provider, for example from the general practitioner. I do my best to ensure your privacy. This means, among other things, that I:
    Handle your personal and medical data with care; Make sure that unauthorized persons do not have access to your data. As your treating coach/therapist, I have sole access to the data in your file. In the event of my sudden death or serious illness, an authorized observer will manage my affairs and have access to your details. I have a legal duty of confidentiality (medical professional secrecy).

4. The data from your file can also be used for the following purposes:

    To inform other care providers, for example when the coaching/therapy has been completed or when a referral is made to another practitioner. This only happens with your explicit (written) permission. For use for observation, during my absence. In consultation with you. For anonymous use during peer review. A small part of the data from your file is used for the financial administration, so that I or my administrator can draw up an invoice.

5. If I want to use your data for another reason, I will first inform you and explicitly ask for your permission.

6. You have the right to inspect your client file at all times.

7. You have the right to have your data removed at any time.

8. The data in your client file will be kept for 20 years, as required by law (WGBO).

9. If you use the services of practice CeeYu, you provide your personal data in order to provide you with these services. This can be done, for example, during a conversation, via the website, via email, by telephone or in another way. It is also possible that your personal data is obtained via third parties in the context of, for example, an external coaching process.

11. Privacy on the care bill/invoice: The care bill you receive contains the information requested by the health insurer, so that you can declare this bill to your health insurer:

12. Your personal data could also be provided in the context of a legal obligation or court order. Praktijk CeeYu will not provide your data without your consent or otherwise anonymised.

13. Your data will not be provided for commercial or charitable purposes.

14. In addition, your data may be stored by third parties outside the EU when you use Google Analytics, LinkedIn or Facebook to contact me. These parties are "EU-US Privacy Shield" certified, so that they will comply with European privacy regulations.

16. You have the right to request access to your personal data. After receiving your request, you will receive an overview of your personal data within 1 month. If this shows inaccuracies, you can request that your data be adjusted, supplemented, deleted or protected.

17. You can also request the transfer of your personal data or you can object to the processing of your personal data due to special personal circumstances.
